All service users accommodated by A New Leaf will be reviewed and monitored regularly. A date of expected achievements i.e. aims and objectives will be set from the point of the initial assessment and follow up actions will be carried out to measure how well the objectives are being reached.

Supported Living
A new leaf Accommodation Provision
The emphasis and Ethos of A new Leaf is to work with service users assessed as having Autism, Aspergers or a learning Disability diagnosis. The functions of the Accommodation is to provide staff to support and enable the service user to develop life skills, boost their morale and build up their self esteem in order that they can function as independently as possible in the future.
The accommodation is designed to accommodate service users from the age of 18 upwards. Some of the service users that will be assessed as being suitable for A new leaf accommodation have been at a disadvantage from an early stage in life and as a result they have never had the opportunity to reside in an accommodation that was not regimented.
Staff from A new Leaf will work closely and in partnership with the service user, their relatives and carers, Social services, health and other voluntary organisations to draw up individual support plans which will identify unmet needs for the individual and work to address their needs by delivering tailored services:
